Custom vs Over‑the‑Counter Splints – What You Need to Know

A seemingly helpful store-bought splint might actually do more harm than good. In this episode of The Hand‑To‑Shoulder Solution, Carl Petitto examines why over-the-counter splints (OTC) often fail—and how a custom-made orthosis can be a much better option for wrist, thumb, and nerve issues.

🩹 Why Over-The-Counter Splints Can Make Things Worse

  • Flimsy materials with stiffener bars: These bars are meant to keep the splint rigid, but often sit directly over sensitive areas like the carpal tunnel or radial styloid.
  • Pressure on nerves & tendons: That rigid padding compresses the median nerve in the wrist, leading to numbness, tingling, pain, and even atrophy—especially for those with carpal tunnel symptoms.
  • Injuries from seam or stiff bar placement: Rigid thumb splints can dig into the radial styloid, triggering de Quervain’s tenosynovitis.
  • Over-immobilization: Generic splints often lock multiple joints when only one needs support—leading to stiffness, swelling, or tendon-bound complications.

Carl shares real clinic stories—like patients whose symptoms got worse using a store-bought brace and improved once removed or swapped for custom options.

🛠️ The Custom Orthosis Advantage

Instead of off-the-shelf designs, Carl explains his 20-minute molding process in the clinic:

  1. Trace & cut heat-moldable material
  2. Mold it directly to fit wrist or thumb anatomy
  3. Add bubbles for bony prominences
    • This prevents pressure over radial styloid or carpal tunnel
  4. Add proper padding and straps
    • Secure but not compressive—avoid blood flow or nerve compromise

Result: a splint that immobilizes only the necessary joints, while staying comfortable, protective, and effective.

🙌 When Off-The-Shelf Splints Might Be Okay

Carl doesn’t dismiss every store-bought brace—just the rigid, ill-fitting ones. A light compression glove, for example, is okay for arthritis if worn properly without extra wrist straps that can compress important structures. The key is cautious selection and fitting.

✅ Takeaway Tips for Patients & Clinicians

  • Avoid hard splints that put pressure on sensitive nerve/tendon structures
  • A custom orthosis offers superior fit, comfort, and protection
  • Quick molding—20 minutes in-office—can prevent complications and speed healing
  • Even simple compression gloves can be useful—if fitted properly
